Today, on 32nd day of hunger strike at Parliament Square London, I, Len Miskulin of sound mind and fully aware of the gravity of the situation and my decisions hereby declare:
Under no circumstances do I give permission or right to any person/ persons to make decisions on my behalf regardless of my condition. In case that I collapse / fall unconscious I agree only to be taken to hospital and connected to saline drip. I strictly forbid any attempts to feed me, either orally or intravenously.
I am NOT an organ donor and therefore it is my explicit demand that my body is not tampered with. In case of my total incapacity my next of keen is noted in my passport and copy of this declaration will be kept in my passport as well as published at:

Only exception to above is if the government announces publicly immediate agreement to deal with my request as outlined in separate document at:

In which case I do agree to IV feed and any other necessary action to revive me.
